What is bariatric revision surgery?
Bariatric revision surgery changes or replaces an earlier weight-loss surgery. People consider it for weight regain, not losing enough weight or reflux. A revision is more complex than a first surgery, which is why your surgeon reviews your records, tests and goals first.
How does revision surgery work?
It depends on your first surgery. Dr. Edgar Serrano's revision page lists six pathways: sleeve to Roux-en-Y bypass, sleeve to SADI-S, band to sleeve, band to bypass, re-sleeve in selected cases after testing, and a pouch or outlet reset for selected bypass patients. Which option fits, if any, depends on testing. Sometimes no surgery is the right choice.
Who is revision surgery usually for?
Revision surgery is for people who've had weight-loss surgery and now have weight regain, too little weight loss or reflux. There's no single cutoff. Your surgeon reviews your records, your earlier operation and your health today, then decides whether a revision makes sense, and which kind.
What do studies say about revision?
After a sleeve, converting to gastric bypass is the most common revision. Your surgeon explains the trade-offs.
Common options after a sleeve
| Option | How it works | What studies say |
|---|---|---|
| Sleeve to gastric bypass | Small pouch joined lower in the small intestine | The most common revision after a sleeve |

How does revision compare with other options?
Each revision ends in one of the other procedures, so its trade-offs follow that target. Converting a sleeve to gastric bypass is the option most often discussed for reflux. SADI-S needs supplements for life, and duodenal switch needs long-term supplements as your surgeon directs, both with regular lab checks. Compare each procedure below, then ask your surgeon which path fits your case.
| Procedure | How it works | Reroutes intestine? | Reversible? |
|---|---|---|---|
| Gastric sleeve | Removes most of the stomach; what's left is a narrow tube | No | No |
| Dr. Serrano's One-Port Sleeve | The same sleeve, through one small incision, usually at the navel | No | No |
| Gastric bypass | Small pouch joined lower in the small intestine; two connections | Yes | Difficult |
| Mini gastric bypass | Long, narrow pouch; one connection | Yes | Can be converted, or sometimes reversed, as a new operation |
| Duodenal switch | Sleeve plus an intestinal bypass; two connections | Yes | Ask your surgeon |
| SADI-S | Sleeve plus an intestinal bypass; one connection | Yes | Ask your surgeon |
| Revision | Changes or converts an earlier surgery | Depends on the plan | Depends on the plan |

How long is the stay and recovery?
How long you stay depends on the operation your surgeon plans. Your surgeon's team confirms your hospital stay, your travel home and when you can return to work. Plan on about four to six weeks before heavy lifting, and only once your surgeon clears you.
If you need a pre-op diet, your surgeon's team gives you a plan.
Your trip, in order:
- Pickup in San Diego and transportation to Tijuana
- Tests, then your surgery and hospital stay, as your surgeon plans
- A driver from the transportation company we hire picks you up in San Diego and drops you right back.
- Follow-up and nutrition guidance after you return home
Your diet moves in stages over several weeks, per your surgeon's plan, and you'll take vitamins and minerals as your surgeon's team prescribes.
What happens after you're home?
After surgery, Dr. Edgar Serrano's team gives you nutrition and recovery guidance. Weight-loss surgery needs regular follow-up, vitamins and lab checks, so before you travel, plan which doctor will see you at home, and bring copies of your surgery records to that first visit.

Revision surgery FAQ
How much does revision surgery cost, and how can I pay?
Revision surgery is quoted after your surgeon reviews your case, so there's no set price. You can pay by Zelle, cash, or cashier's check. Your written plan shows your total for each option and lists what's included up front.
What records do I need for a revision review?
Gather your earlier operative report, recent lab work and any imaging or endoscopy results you have. Your surgeon may ask for more tests, such as an upper endoscopy or a contrast study, to see your current anatomy. Our coordinators help you send everything so your surgeon can review your case and build your plan.
Can a sleeve be converted to a gastric bypass?
Yes, it's the most common revision after a sleeve. A 2025 study of more than 33,000 revisions after a sleeve found most were conversions to gastric bypass. It's often discussed for reflux or weight regain. Your surgeon reviews your records and tests first, then explains whether it's the right path for you.
How long will I stay in Tijuana for a revision?
Your plan, stay and price are set after your surgeon reviews your case. A revision can differ a lot from one person to the next, so there's no standard schedule to quote. Once your plan is set, our coordinators lay out your trip in writing, from pickup in San Diego to your ride to the airport.
How much does lap-band removal cost?
Lap-band removal is priced by personalized quote once your surgeon reviews your case, like other revision surgery. Dr. Edgar Serrano's revision page lists band removal among the factors that can affect your written quote. Your written plan shows your total for each payment method.
How do I choose a revision surgeon?
Ask how often the surgeon does revisions like yours and which tests they need before deciding. Ask why they recommend one option over another, including no surgery. Ask how you'll get your records and what follow-up looks like at home. Your consultation with the surgeon is a good time to ask.
